Information in an inquiry
The form collects your city or postal code, requested service, professional category, preferred timing, name, email, phone and optional contact preferences. Do not include insurance numbers, medical records or detailed health histories.
Purpose, storage and sharing
With your consent, ORAL stores the inquiry in its hosted database so authorized operators can handle it and contact you. An inquiry is not public. The site does not automatically forward it to a provider. Provider sharing requires separate consent. Hosting infrastructure may process information outside your province or Canada.
Access, correction and deletion
Use the contact inquiry and choose “Privacy request” to request access, correction, withdrawal of consent or deletion. Include your inquiry reference if available, but no identity documents. ORAL may need to verify your identity before acting.
Technical data
Hosting services process technical logs. The form uses a short-lived hashed network identifier to limit abuse; it is not used to advertise to you. No advertising tracker or third-party analytics vendor is installed. Form contents are not sent through the site’s analytics event hooks.
